To apply for a Massage Establishment Permit, the business owner will be required apply for an annual Livermore Police Department (LPD) permit.  If approved by LPD, the business owner may then apply for a business permit through the City of Livermore.

Download and complete the following documents:

1.  LPD Massage Establishment Permit Application
Complete all requirements set forth in application before submission to LPD.

2.  Livermore Municipal Code 5.28
Review code and sign last page in acknowledgement.

3.  Live Scan Form
Take this form to a Live Scan provider of your choice.  Retain a receipt/proof of service and submit to LPD with above forms.  *Results will be automatically sent to LPD only if this form is used.
